These rates reflect a 10% discount off the standard rate for payment at the time of service.
I am currently in‐network with Pacific Source. I will be in‐network with Kaiser starting in April 2024.
You may be eligible for out‐of‐network benefits. I can provide you with a Superbill (receipt) that you can use to process your reimbursements directly.
If you need to cancel your appointment, please do so within 24 hours of our appointment to avoid a late cancellation fee of $100.
No‐show appointments will be charged at the full fee of $180.
